If I had to bet, hpz3r4v2.dll could be an HP printer driver, so try reinstalling the printer driver on this PC, if it's an HP printer. But go to HP's site first and see if there's an updated driver for this printer for whichever version of windows you use.

Also, close Word and search the hard drive, including hidden and system files, for the template normal.dot. Delete all copies of normal.dot that you find. When you restart Word, it'll create a new copy of normal.dot
