Just in case someone else search's for this issue, here's what it was for us...
It seems as though the secondary IDE channel that the DVD-ROM was connected to has failed, maybe. All I know is that I don't get the errors when I move the ribbon to the primary IDE channel. The real weird thing IMO though is that the drive worked fine to restore our PowerQuest image 3 times during this troubleshooting process. And the image doesn't cause this issue on any of our other 340's.
At any rate, try moving the ribbon to another IDE channel. That worked for us.
