SupportAssist OS recovery: Reset Fail

Blue screen issues: I boot and under Windows Boot Manager, i select SupportAssist OS recovery
No issues with Hard Drive scan ... so far so good
I select: Reset to factory settings
no back up of files Factory reset in progress 

runs smoothly until it reaches the Clearing Partition section
Check Partition: works well
Format Partition: turns into a black screen and says: Reset Failed!

what to do next ?

Most likely an SSD/HDD failure. Dell's hardware scan may not pick all type of HDD failures. 

in BIOS > Storage > Sata operation: Change to AHCI and try again. 

If still no go a clean format will be needed. 

Use an 8GB pen drive to create windows bootable media and perform an operating system reinstallation.
