First see if you can uninstall EQAdvice by going to Start, Control Panel, Add/Remove Programs.

Then

Get the latest version of ccleaner from:
 
from http://www.ccleaner.com.
 
(the actual download is at: http://www.filehippo.com/download_ccleaner/
click on on Download Latest Version)
 
Install it.  Don't let it clean anything yet. 

Download the killbox:

http://www.bleepingcomputer.com/files/killbox.php

Unzip it to your desktop but don't run it.

Shutdown and Restart and Boot into Safe Mode by tapping the F8 key when you see the PC
maker's logo.
Keep tapping until it tells you it is going to Safe Mode or you see the Safe
Mode menu. Select the top option. Log in as your usual login or you won't find the programs you put on the desktop
and some of the entries we want to remove will not appear in HijackTHis.

Run HijackThis and just do a Scan only. Check  then Fix Checked any of the following that remain:

Run ccleaner.exe,
 
Select Options then Advanced and uncheck the box in front of:
Only Delete file in Windows Temp folders older than 48 hours.
Now select Cleaner
Under Cleaner Settings, Windows
 uncheck everything on the first page
except:
 under Internet Explorer
   - Temporary Internet Files
 under System
  - Empty Recycle Bin
  - Temporary Files
Under Cleaner Settings, Applications uncheck everything
except:
 Under Internet
  - Sun Java
Run Cleaner.
 
This should clean out all of the temp files including those of your java program (where recently we are finding a lot of garbage.  You really should be running the latest version of java and uninstall all old versions).  The reason I have you uncheck most of the options is that I have had problems with it  deleting too much so I want to limit it to things where I think malware might be hiding.
 

Run killbox.  Open Options and check Remove Directories
Where it says Full Path of File to Delete you need to type or [copy (Hightlight and Ctrl + c)
and Paste (move to the killbox and place the cursor in the box and Ctrl + V)]:
 
C:\Program Files\outlook
 
Then check the Delete on Reboot box then the red button. 
It will say:  File Will Be Removed On Reboot, Do you want to reboot Now.
Tell it NO.  (If it can't find it that's OK just go on to the next one)
 
Repeat for:
 
C:\Program Files\EQAdvice

C:\WINDOWS\system32\Runner.dll
C:\WINDOWS\system32\cdofpfnc.dll
C:\WINDOWS\system32\cmstart.dll

Let it reboot after the last one.

Reboot into regular mode maken a new HJT log and post it as a reply.

my new log file...my mouse is acting up now and my desktop won't show my wallpaper. the wallpaper shows up when the computer is loading, but as soon as my desktop icons load, the wallpaper disappears and all you see is the solid color background. any suggestions? thanks for your help...please let me know if i need to fix anything else in the new log file. i really appreciate all the help!

Log is clear now but It's possible that you have one of the smitfraud variations.  Get smitrem from Dave's World and run it.
 
 
(Better, more detailed, instructions are available at bleepingcomputer.  (They are fighting spyaxe which you don't have but they run smitrem as part of the procedure.)
 
 
If smitrem doesn't fix it then try:
 
Right click on your desktop and select properties.  Look to see where the file is and what name it has.  If it ends in .html then it's one we've see before.  Usually it's in C:\Windows or C:\windows\system32
 
Right click on Start and select Explore and then Tools, Folder Options, View, and uncheck the two boxes where it starts with Hide and check the box that says Show Hidden Files and Folders (ignore the stupid warnings) OK. 
Download a new wininet.dll from
and extract it to C:\
Use Windows Explorer (Right click on Start and select Explore ) to verify that the file is present in the right pane when you click on Local Disk C: in the left pane. 
then boot into Safe Mode with Command Prompt.  Type the following with an Enter after each line.  Do not type the stuff in ( )'s.
cd \windows\system32 (there is a space after cd)
del /f dllcache\wininet.dll  (there is a space after del and after /f it may not find it there)
del /f wininet.dll  (there is a space after del and after /f )
copy \wininet.dll (there is a space after copy)
 
Reboot.  (Ctrl + Alt + Del and select Shutdown)
 
Run killbox and have it delete the file you found as the source of your desktop. 
 
smitrem's runthis.bat makes a log that I think it stores in its folder.  Post the log as a reply.
