This looks like Hyper-V, what version of Hyper-V are you trying to (re)provision on?

Are you using vWorkspace Instant provisioning?

On the actual Hyper-V server itself in the Hyper-V manager, do you see more than one VM of the same name?

Do you know if you have the following mandatory hotfix installed on all your brokers?
