Looks like you may have multiple issues from viruses to malware to spyware.
http://www.microsoft.com/security/malwareremove/default.mspx
This will help to identify and delete malware. Run your anti-virus software and then you might want to post over on the HJT forum.

The only other way to be sure would be to reinstall XP. That is a clean install.
