I need help

Something has been going wrong with my computer lately so I reformated it to see if that would resolve the problem.  But the same problem was there. 
The problem is :
1. Whenever I try to install something it will go "Not Responding" and won't go back to normal and I have to restart computer or try to exit it.
2. Whenever I press Control+Alt+Delete the Task Manager appears and disappears like something exits it everytime. (When windows is first started and I press it it comes up and after some program loads (don't know which) the program deletes it?)
3. Some programs wont even load when I click on them. (EX: when i go to Start pop won?t window the msconfig in type and>
 
I have tried looking through regedit and stuff but I didn't find anything. When I start it into Safe Mode everything works fine (msconfig and task manager) but when I try to see if they work in regular mode they wont.
 
Can someone please help me with this problem.
Remember I cannot access some (maybe most) programs on my computer.
 
Edit: I have a demension 2400
Edit #2: I was looking through some other posts and I came across another error that I forgot about.
Then I also have this windows blue screen of death error "Stop 0X00000000A" in which the system crashed altogether.
This error annoys me a lot because it happens quite often for me.  Is there something wrong with my computer?

Chances are you have a virus or worm. Do you have current antivirus software? If so, scan the system thoroughly in Safe Mode.

Yes I have scanned my computer and I have reformated.

I dont know if you have tried all kinds of virus and spyware sweeps but i would hit it with every one you can.  Sometimes one will pick up something others wont. Try adaware(free), spybot(free), microsoft beta spyware(free if good xp codes), spysweeper( free for 30 days),  even a yahoo toolbar has one too.  I think you can try the hijackthis on the support home page too.
