Out Of Scan Range Issues After Automatic Driver Update.
On June 8, 2005, an updated chipset for the systems with the Intel 915 chipset was marked as a Critical Update on Windows Update. When it is installed, it sets the default mode to 1280x1024, which is beyond the capabilities of some of the lower end flat panel displays and certain older CRT monitors. If the system starts getting an "Out of Scan Range" or "Cannot display this video mode" error after Automatic Updates installed something on the system, follow the instructions below:
1. Boot into Safe Mode and lower the screen resolution to 1024x768.
2. Restart the system and then boot into the Windows Advanced Options Menu and the Enable VGA Mode. The system will load normally.
3. Browse through
http://support.dell.com/support/downloads/index.aspx
and then choose your system type, video drivers and then Windows XP for the operating system.
4. Choose Intel 915G Graphics Controller or the Intel 910 Graphics Controller and then click the most recent driver from the list.
5. Click the driver name and the download it using HTTP and the run the application. Follow the instructions.
This should fix the issue for the mean time.

I saw a post addressing a similar problem on the OptiPlex board.
Looks like Microsoft posted a bad driver. It's been pulled I believe.
