I have tried this in every possible combination I could think of, no change

All joking aside, are there any known problems with the USB ports on this monitor? If not, then point me to the latest drivers and I will reinstall them (one last time). If that doesn't work, I will assume the monitor is defective and contact support.

I had a similar problem w/ my 1704FPV. I did all those steps mentioned above to no avail. Even tried tweaking some USB related things in the registry which failed. When I disconnected and reconnected the upstream cable from the monitor I would get the unrecognized and has malfunctioned error also. So the other day I was reading a Microsoft support article about how sometimes when USB ports don't receive enough power they dont function properly. That gave me the idea to switch the monitor off, unplug the power supply from the monitor, and plug it back in, and WAH-LAH! the ports worked again. I quickly switched on the monitor to notice that the correct drivers were being installed (the bubbles in the taskbar in XP). Checking the device manager there no longer was an 'unknown device' but a 'Generic USB hub'. I don't know why, but that simple step fixed my problem. Hopefully that works for you also. Thanks for the tip, but it did not work. I turned off the monitor, unplugged the monitor, waited 5 minutes (chanting the 'Gita at the same time). When I repowered and turned on the monitor, I got the Found New Hardware balloon, but it was the same 'Unrecognized device or malfunctioning ..." message.
Any other recommendations? Otherwise, who should I call for the support exchange?
Now did you unplug from the outlet, or from the monitor? Because I unplugged from the monitor and I figured something was loose on that end that somehow kept the monitor running but not the USB hub. I also did it while the PC was on, and unplugged and plugged almost immediately, so no need to wait 5 min.
Only other thing I tried before the unplugging idea was to uninstall everything under Universal Serial Bus controllers in the Device Manager (Right click 'My Computer'>Properties>Hardware Tab>Device Manager button) and restarted. Restarting would cause a fresh copy to be installed of all the USB controllers.
