Have you tried uninstalling and reinstalling the Sigmatel STAC9200 High Definition audi driver, and have you run the Dell hardware diagnostics?
 
Also, which version of the Creative drivers are you using? From Dells support website, from the "integrated Sound Blaster Audigy ADVANCED HD audio" CD, or from Creatives website?
 
Also, do you use Bluetooth/USB headphones or some other external audio device with your notebook other than standard stereo headphones plugged into the headphone jack?

I just ran Dell Hardware Diagnostics and the Interrupt Test and the Analog Sound Playback Test failed.  However, last night it randomly started working again.  Then when I woke up and started it up this morning it wasn't working again.  I have the Sound Blaster Audigy Advanced HD cd, it came with the laptop.  I also use Logitech speakers but the problem seems to occur whether these are plugged in or not.  It seems like whenever I boot up my computer it will just either work or not work.

Even with the Creative integrated Sound Blaster Audigy ADVANCED HD Audio CD, you *still* need to install the Sigmatel 9200 HD audio driver first. Without it, the Sound Blaster Audigy software will not function correctly.  However, sind you received a hardware error during the Dell diagnostics...
 
If the hardware tests fail even once, then there is a hardware problem. Did it give you an error code when it failed?  If so, did you write it down? 
 
I suggest you run the hardware diagnostic again when it fails and write down the entire error message (or do what I do, use a high resolution digital camera to take a picture of the screen so I can read it back later)
 
Then, call Dell technical support to get it fixed.  I hope you are under warranty :)
