Problem solved, but see answers below for reference.

I saw a very similar issue reported on Intel's community forums (search for Re: Intel SSD 530 startup problems - Bug in SSD?).  The problem was traced to a conflict the Intel Rapid Storage Technology and an LSI SandForce controller was referenced.  The Kingston V300 uses an LSI SandForce controller and the Dell E6540 Intel chipset does provide IRST.

Every time I had tried installing the Intel IRST drivers provided by Dell, I received an error stating that .NET 4.5 Framework must be installed.  Note the IRST drivers are downloadable from the SATA section of Dell's drivers list, not the chipset section.  I had never bothered installing the 4.5 .NET Framework myself.  

Downloading the framework from Microsoft, installing it, and then installing the SATA IRST driver (Serial-ATA_Driver_R4HDM_WN_13.1.0.1058_A11.EXE) solved my problem after the first reboot.  

1. No USB, docking station or external devices connected. 

2. Yes, SSD health is good per manufacturer's assessment program. 

3. Only happens with the SSD.  No other HDD causes the issue.

It seems that I consistently run into this problem on a "warm boot" where the power to the laptop has not been completely switched off.  

There were two cases where the problem was a bit more involved that that, which caused me to ignore the pattern for a while but both of those cases were during a Windows update that had gone wrong in some way, from what I can tell.

1. Do you have any USB devices plugged in during boot? A docking station? If yes, try to boot without any of that.

2. Have you verified the health of your SSD?

3. if it keep freezing at the Dell logo remove the SSD and do a few power on/off cycles to see if its still happening or try another hdd/ssd.
