Connectrix: Brocade: Switch enable failed. switch status = 0x4

Summary: Post performing hareboot or hafailover, switch was showing error "Switch is not in ready state. Switch enable failed. switch status = 0x4". Switch was in Disabled state.

Symptoms

Performed "hareboot" on the Departmental switch and Switch was stuck in Disabled state. Ping works fine and user can execute the commands. 
Could not get the Switch online even after rebooting the switch using command, Power reboot (disconnect and connect power cord) and install or upgrade the Firmware again. 
It is possible that Firmware upgrade starts but does not get committed.

Cause

Issue was caused as switch was tried to access from multiple places or multiple sessions (Telnet or SSH or Webtools) at the same time and one of the user performed "hareboot". 

Resolution

Switch needs to be replaced.
