Left mouse click ossacionaly does not work after rebooting

I have a DELL XPS 400 with a standard 2-button, plus roller, optical mouse.  The problem occurs sporadicly and seems to happen only at reboot.

Is this likely a hardware problems or a sortware or driver problem?  Where should I start looking for the problems, or what steps can I take to dianose the problems.

The best way to troubleshoot would to have another mouse and see if that also does it.  You can buy a "test" mouse relatively cheap.  It could be the Windows driver, or the USB port but I would rather suspect the mouse.  You can check the USB port by plugging it into a different port and see what happens.  On many models there are USB ports that are active at boot up only, and you should normally have the mouse and keyboard plugged into those ports but for testing you can plug it into any port.
