m15 R4, constant stuttering in Advanced Optimus or dGPU only modes

A few weeks ago I received the now unavailable(?) m15 R4 i7 w/3080 and the 1080p 144Hz screen with Advanced Optimus.
I've never been able to play games using Advanced Optimus ("Automatic Select" in Display Mode, in the Nvidia Control Panel). While the frame rate is consistently high in all games (300+ FPS in some cases), there's constant stuttering and micro-stuttering that makes games unplayable. This is evident in all games I've tried.
Exactly the same issue presents when using the "NVIDIA GPU only" mode. Games are unplayable in dGPU only mode. Temps on the GPU and CPU are fine, often below 70c.
If I use "Optimus" mode (OG Optimus), the stuttering goes away but OG Optimus doesn't support G-Sync so when frame rates dip below 144 FPS, I get really obvious tearing.
Advanced Optimus and G-Sync are the main reasons I choose this laptop against other options but it's not possible for me to use either.
I've tried:
- Restoring OS via SupportAssist x2 times
- Clean install of Windows 10 x2 times
- Dell's Nvidia driver (461.66)
- Nvidia's latest driver (466.27)
- Latest iGPU driver
- Windows updated to latest or rolled back to November 2020
- Various different power/performance profiles
- Made sure no external screen connected
- ...
- Many other things
I can't play games on three of my external monitors using V-Sync without locking the clock speed of the dGPU, seemingly due to driver incompatibility issues with these older screens, which just adds to the frustration.
I'm seriously beginning to wonder if this i7/3080/144Hz screen option is no longer available because there are inherent compatibility issues that Dell is aware of.
Any insight or advice would be very welcome.
