Performance Issue when use heavy task or gaming. It not charging and when it above 92% it drop fps all drop

I'm using a Dell laptop and it doesn't draw full power when I play games or run heavy tasks. When the battery drops to 91%, the system starts throttling, which causes the laptop to lag.
I checked with HWiNFO, and when I’m gaming the system basically uses only the AC adapter power and not charging. My laptop is showing the LED code 3 amber and 8 white, so I’m not sure what issue it’s having. From what I’ve researched, this model supports Hybrid Power Design, but it shouldn’t be throttling — it should just use both the battery and the AC adapter together to provide enough power
I use dell precision 7560

Thank you for your answer to my question. However, I just replaced the battery and BIOS reports that its condition is excellent. I don’t know how to check the power source. When you mentioned ‘power’, do you mean the AC adapter or the internal power circuit? Because I already replaced the battery completely.

I’m thinking about switching to a 240W charger if possible. If there is any way to check it, could you please let me know?
