AppSync: Unable to configure SNMP Notifications for alerts

Symptoms
To Configure SNMP notifications, go to Alerts > SNMP Notifications. Click Add. Select the desired trap destination from the Destination drop down.
However, it remains in the "No data available" state, and the settings are not being applied.
