PowerEdge R210II RAID 1 H200 2008R2

I have a R210 II running Windows Server 2008 R2 with RAID 1 on an H200 controller with 2 x 500GB hard disks. The single virtual disk shows status as permanently degraded, and in OpenManage Server Administrator, it indicated a failed disk 0:0.

I replaced the failed disk with a new one. The rebuild gets to 47% and then stops, showing a status of Permanently Degraded once again. If I reboot, I see the same thing - rebuilding that gets as far as 47% and then stops.

I have run CHKDSK on the logical disks, and no errors are found. Could it be the H200 PERC controller causing the problem?

Any suggestions very welcome!
