Have you recently updated your storage controller firmware? What is your current version of the storage manager? You may try uninstalling MDSM, and install the latest downloaded from the support site.
No updates to the storage controller firmware recently. When I try to update the firmware I have tried multiple versions of MDSM, the one currently installed is 11.10.0G06.0005.
I have tried the latest version available but still no luck.
