I could recommend a few initial checks:  check Firmware and/or a loose connection. Reseat all cables and connections and update the firmware (controller, MDSM and hard drives).

 

Was the replacement drive listed on the support matrix?

• Suggestion to have a verified backup based on number of old failing drives
• Determine if 0:5, 0:19, 0:22 were physically replaced on 26th and 30th
• • If not physically replaced,
  • • Replace them safely
• Replace 0:10, 0.13
