Cause
The most likely cause is that the SP is miscabled.
The neo14:60196 error indicates that either the cables or the ID of the disk array enclosure (DAE) has been misconfigured. If the system was indeed in a period of maintenance and attempting to come back online, there is a good chance that the cables are not connected properly.
This error will be accompanied by other errors, like neo14:60258 (drive offline) and neo14:600F4 (filesystem offline) which are all results of the SP being miscabled.
