Connectrix MDS: Failed Upgrade on MDS 9706 or 9710 with Error "bootflash busy; seg fault while mounting bootflash"

Symptoms

Upgrading from NX-OS 6.2.19 to 8.1.1.

After upgrade, user is no longer able to see "bootflash://sup-standby."

Seen as follows: 
MDS1# dir bootflash:
bootflash:///
bootflash://module-5/
bootflash://sup-1/
bootflash://sup-active/
bootflash://sup-local/ 
A working switch shows the following: 
MDS2# dir bootflash:
bootflash:///
bootflash://module-6/
bootflash://sup-2/
bootflash://sup-local/
bootflash://sup-standby/
bootflash://module-5/
bootflash://sup-1/
bootflash://sup-active/
bootflash://sup-remote/    <---------------
Other symptoms of the issue can be the following:
  • ISSU/D fails with the following error message:
Pre-upgrade check failed. Return code 0x4093007E (Issue cannot be done, bootflash is busy). 'segmentation fault' error messages when issuing `sys health check bootflash`:

switch# sys health check bootflash Unmount successful... Checking any file system errors...Please be patient... /isanboot/sbin/format-bootflash: line 75: 7598 Segmentation fault $SUDO $E2FSCK -n -v $@ > /dev/null 2> /dev/null done. 
Remounting bootflash .../isanboot/sbin/format-bootflash: line 62: 7600 Segmentation fault $SUDO /bin/mount $MNT_BOOTFLASH_DIR Failed to mount bootflash bootflash on the affected supervisor is not available switch# dir no such file or directory (invalid server) 
Logging log outputs: %BOOTVAR-2-AUTOCOPY_STANDBY_MOUNT_FAILED: Autocopy failed to access standby file system via NFS mount.Please, copy the boot images manually to standby bootflash. %SYSMGR-2-LAST_CORE_BASIC_TRACE: : PID 6688 with message non-sysmgr(non-sysmgr) crashed, core will be saved . Core file also generated: 
`show cores` Module  Instance  Process-name     PID       Date(Year-Month-Day Time) ------  --------  ---------------  --------  ------------------------- 5       1         non-sysmgr       6688      2017-10-08 07:29:07

Cause

The issue is identified as the following from Cisco CSCvb93701: 
ISSU/D fails on MDS 9706/9710 with 'bootflash busy' error; seg fault while mounting bootflash
CSCvb93701 (see also CSCvf99665)
As per the documented Cisco issue: 
The system is only affected if it was previously running one of the following NX-OS releases: 6.2(17) - 6.2(19) 7.3(0)D1(1) - 7.3(1)D1(1) 7.3(0)DY(1) - 7.3(1)DY(1) This issue does not apply to any other NX-OS releases on MDS 9706 and 9710 platforms. It does not occur on the MDS 9718 platform.

Resolution

Workaround:
The IPv6 invalid subnet on the management interface causes this issue. 
switch13# show interface mgmt0

        ::148.125.166.255/0      <--- "0" indicates an invalid subnet
