Symptoms
The disruptive service upgrade and OS Conversion fail if the SVM name in vCenter does not match its hostname:

Cause
The PowerFlex Manager code skips SVMs based on their hostname but is looking at VM name. For example:
Using the VMware plugin, the SVM is named for example ScaleIO-192.168.xx.xx but the hostnames are for example ScaleIO-192-168-xx.xx.
Resolution
Rename the SVM in vCenter to match the hostname.
