Cause

Underlying Issue

The error messages "PDR12 initialization has failed" indicate that the hard drive cannot be initialized because it has failed or is corrupt.

  • Hardware failure of the disk – damage to platters, read/write heads, or internal controller.
  • Corruption of disk firmware or data structures that prevents successful initialization.
  • Environmental or operational factors that can accelerate failure, such as power surges, excessive vibration, or operation outside recommended temperature and humidity ranges.
  • Specific error identifiers that point to the root cause:

Resolution

Replace the failed or corrupt disk. Identify a disk that has failed by locating the disk that has a red "X" for its status. Restart the initialization.
